How to Enroll

How patients can enroll: 

Your patients have four different ways to sign up for Medicaid:

• Enroll by mail or fax, by completing the South Carolina Health Connections form in the enrollment packet and sending it to Health Connections. Call 1-877-552-4642 to get the forms.
• Enroll over the phone, by calling South Carolina Healthy Connections toll free at 1-877-552-4642.
• Enroll online at the South Carolina Healthy Connections website at www.SCchoices.com.
• Enroll in person, by meeting with a community enrollment counselor. Patients should call 1-877-552-4642 to make an appointment.

What is South Carolina Healthy Connections?

Healthy Connections is a new program that helps Medicaid members enroll in health plans.
